There is a moment, somewhere around the forty-fifth minute of a new shift, when a supervisor stops walking and stares at a machine that should not be running the way it is running. Nobody told him. The outgoing shift knew — they had been nursing it for three hours — but the telling got lost in the noise of a busy changeover, a quick joke by the clock-out terminal, a foreman who was already thinking about the drive home. That moment of staring, of not knowing, is not a communication failure in any dramatic sense. It is quieter than that, and far more expensive. What gets left behind is the chronic, the ongoing, the not-yet-resolved. The conveyor belt on Cell 7 that skips under load above 1,200 units per hour. The relief valve on the compressor that has been cycling two minutes faster than the service manual says it should. The temporary workaround that the maintenance technician installed yesterday and marked with blue tape and a note that has since blown onto the floor. These are not dramatic. They are, however, exactly the things that injure people and destroy production targets. The fourth is quality flags: any batch, run, or period where conformance was uncertain, including the exact products and times involved. The fifth is safety events: near-misses, first aids, housekeeping hazards that have not yet been corrected. The sixth is people: absences, anyone operating outside their normal role, a new or returning person who may need closer supervision. The seventh is deferred actions — the things the outgoing shift started but did not finish and that the incoming shift needs to pick up, with enough context to pick them up intelligently.
